Healthy Solutions
is a quality of life improvement program offered free of charge to Gateway
Health Alliance members. If your employer participates in Healthy
Solutions, it is because your company leadership believes that a
healthier workforce is a happier workforce, and wants you to have every
resource available to help you identify and take control of health conditions
that may be limiting your quality of life.
Personal
Health Coach
At risk employees are partnered one-on-one with Healthy Solutions
nurses, who use the latest technology to identify those members most at
risk for diabetes, cancer, asthma, congestive heart disease, hypertension
and high-risk maternity. Participants
are stratified as low, medium or high risk, depending on data gathered
via health risk appraisals and on-site screenings.
Empowered
Employees
An appropriate
follow up plan is crafted for each Healthy Solutions member,
and implemented in a manner convenient for the member. Confidential consultations
with nurses and Gateway Health Coaches, physician referrals, online personal
e-health record, educational materials and periodic newsletters help members
manage risks and optimize the provider/ member relationship.

Some Healthy
Solutions members have added the advantage of technology through
their personal e-health record. Healthgram (www.healthgram.com™)
enhances the teamwork of members, Gateway nurses, and physicians by providing
an individual health profile, Rx history, claims history, and e-links
to medical libraries such as WebMD.

What Difference Does it Make? |
Our approach to health benefits is centered on improving our members'
health. Not all members have become "healthy" overnight, but
we strive to improve the health of even the ones who are very sick. It's
also referred to as a "game of inches." Nevertheless, below
are just two examples of how we've improved quality of life for our members...
386 Healthy
Solutions enrollees at a Gateway client company participated in lifestyle
management strategies for weight loss. A total of 1217 pounds was shed
in under 10 months.
Participants in Baby Plus, Gateway's Maternity Management program, experienced
a 25% reduction in the number of days spent in Neonatal Intensive Care.
