Becoming a Client of a CHW
How to become a client of a CHW:
After your hospital stay or emergency room visit, the care coordinator will call you to ask if you're interested in program participation. If so, a community health worker will set up an appointment to meet with you. The community health worker will share various resources to help ensure you receive the services and support needed to improve your well being. Community health workers do not provide medical coverage, clinical advice, or replace the need to follow up with your doctor.
To see if you qualify for the program, the Community Care Coordinator must review your medical records. Please contact us if you would like to participate in this free program.
